If you need a doctor in an emergency from 6.30pm to 8.00am weekdays, weekends and bank holidays, ring 0845 408 8000 or, for medical advice, ring an NHS Direct nurse on 0845 4647.

Test Results

Please telephone 01278 764244 between 2pm and 4pm for any results of blood tests, x-rays etc. Give your full name, address and date on which your test was carried out.

Practice Nurse – Highbridge Medical Centre


The South Somerset Symphony Project is rebuilding

We currently have a part time vacancy for a Practice Nurse at Highbridge Medical Centre, working 28 hours per week.


Job Summary


The post holder would be part of a nursing team responsible for ensuring that the practice provides optimum care relevant to the practice population, either in practice or the home environment.


The post holder will focus on supporting patients to be healthy, monitoring long-term conditions, health prevention and screening activities in line with the objectives and vision of Symphony Healthcare Services. They will work collaboratively with the whole general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.


The post holder would need to demonstrate relevant experience in Chronic Disease Management: specifically, diabetes and CVD/Hypertension Monitoring.
